U.S. immigration office Form I-693 is a medical examination form that is required for individuals who are seeking to adjust their immigration status to become a permanent resident of the United States. The medical examination must be performed by a U.S. immigration office approved civil surgeon, who will complete the form and provide the results to the applicant.

Medical Examinations

  • *During flu season (beginning of October - end of March) everyone is required to have flu shot for the current season.*
    *Starting January 22, 2025 the USCIS Office is no longer requiring patients of any age to provide proof for COVID-19 Vaccines*
    For children 18 and younger

    They will need to be fully up to date with the required vaccines for their age. Kids 2 years and older are required to have a Tb blood test done during their exam. Please call our office for prices for kids and vaccine requirements since they do vary with age.

  • For people ages 19-24 years

    People of this age will need to have proof of vaccines for Tdap (with in the last ten years), Polio (at least one shot), MMR, Hepatitis B(at least one shot), in order to schedule. During their visit they will complete a TB blood test, Syphilis blood test, and a urine test for gonorrhea with the civil surgeon. These test are required to be done by the civil surgeon.

  • For people ages 25-44 years

    For people ages 25-44 years old will need to have proof of records for Tdap/Td (with in the last ten years), Polio (at least one shot), MMR, Hepatitis B (at least one shot). During their visit they will complete a TB blood test, Syphilis blood test with the civil surgeon. These test are required to be done by the civil surgeon.

  • For people 45-59 years

    People of this age will need to have records of Tdap/Td (with in the last ten years), Polio (at least one shot), MMR, Hepatitis B (at least one shot). During this examine patients will only need to do TB blood test. This test is required to be done by the civil surgeon.

  • For people 60-64 years

    People of this age will need to have records of Tdap/Td (with in the last ten years), Polio (at least one shot), MMR. During this examine patients will only need to do TB blood test. This test is required to be done by the civil surgeon.

  • For people 65 years and above

    People of this age will need to have records for Tdap/Td (with in the last ten years), Polio (at least one shot), Pneumococcal. An MMR is required if they were born in 1957 or later. During this examine patients will only need to do TB blood test. This test is required to be done by the civil surgeon.

IMPORTANT: Vaccine requirements should properly correlate with the patients’ age on the day of their appointment. Age change can impact medical requirements, such as the age at which an individual is eligible for certain vaccines and or medical procedures. Individuals should be aware of these changes to ensure that they are in compliance with applicable laws and regulations.
