U.S. immigration office Form I-693 is a medical examination form that is required for individuals who are seeking to adjust their immigration status to become a permanent resident of the United States. The medical examination must be performed by a U.S. immigration office approved civil surgeon, who will complete the form and provide the results to the applicant.
They will need to be fully up to date with the required vaccines for their age. Kids 6 months and older are also required to have at least two COVID-19 vaccines in order to schedule. Kids 2 years and older are also required to have a Tb blood test done during their exam. Please call our office for prices for kids since they do vary with age.
People of this age will need to have proof of records for Tdap (with in the last ten years), Polio (at least one shot), MMR, Hepatitis B(at least one shot), and a minimum of two doses of COVID-19 vaccine in order to schedule. They will need to do TB blood test, Syphilis blood test, and a urine gonorrhea test for gonorrhea.
